- Collaborate with medical providers, government agencies, and other private entities to achieve client success in maintaining housing and stabilizing mental health symptoms
- Provide direct/ indirect services to individuals who are housed via government subsidy in Redwood City by conducting 1-2 individual assessments that report the housing situation (unhoused, housed, risk of losing housing) of an individual along with assessing access to resources for improving quality of life that result in actions to decrease barriers to resources
- Advocate for individuals to increase access to necessary resources and community services to improve quality of life
- Deliver over 30 individualized service plans created with individuals to identify areas of high need in which interventions are implemented to remove barriers to resources and services (referrals or connect to services directly
- Utilize evidence-based approaches (harm reduction, permanent supportive housing, and motivational interviewing) to better understand individuals needs resulting in treatment planning and implementation
