    - Provide operational and clinical staffing for a large rehabilitation facility, dispensing and preparing medications for delivery to units as well as providing clinical interventions and collaboration as part of the healthcare team - Complete and document a thorough medication reconciliation upon admission, reviewing the discharge summary from the previous facility and ensuring that all orders are appropriate and that a drug utilization review is completed to monitor for interactions, duplications, allergies, or potential formulary substitutions - Review charts monthly for two assigned units ( 100 patients), ensuring that medications are dosed and administered - Verifying the order entry and dispensing of a one-week supply of medications to be delivered to the patient unit - Provide practical, evidence-based drug information for providers and nursing staff on various medication related issues such as compatibility, dosing, and adverse reactions - Monitor patient laboratory values and provider notes in order to recommend clinical interventions such as renal dose - Collaborate and communicate with members of the healthcare team to ensure that patients within the facilities receive wellrounded - Review and assess laboratory results and other clinical information to appropriately dose and monitor medication regimens such as antibiotic and anticoagulation - Provide pharmacokinetic dosing recommendations for vancomycin and aminoglycoside medications, assessing renal - Manage the operational workflow of the pharmacy, ensuring efficiency and safety while ensuring that orders are completed and delivered to the unit in a timely manner - Perform sterile compounding using aseptic technique in accordance with current <USP 797> guidelines - Maintain a therapeutic interchange list to optimize patient care while reducing medication costs - Provide antibiotic recommendations as requested by providers on issues such as antibiotic selection and dosage - Review patient medication profiles for appropriateness of drug therapy, therapeutic duplications, allergies, weight, height - Assist patients at discharge by reviewing the discharge summary and dispensing a one-month supply of home medications and delivering to the unit with an opportunity for discharge counseling - Oversee third-party insurance billing for patients within the long-term nursing facility on campus, ensuring that medications - Manage pharmacy inventory, submitting a daily order to ensure that the pharmacy stocks adequate supplies of medications without excessive overhead costs
