Do you now or will you in the future require work authorization or a visa to work in the US (e.g. H-1B status, TN status, O-1 status, L-2S dependent status, Employment Authorization Document (EAD), OPT/CPT or any other non-immigrant status)? For example: Are you currently in the U.S. on an H-1B, F-1, OPT/CPT, TN, J, or L or any other employer-specific non-immigrant status? Do you have an EAD tied to a nonimmigrant visa which cannot be renewed beyond a specific date? Do you have an EAD issued in connection with an application for permanent residence?

Are you legally authorized to work in the U.S. beyond a practical training period? (Please note that it is an employment requirement to provide proof sufficient to satisfy U.S. immigration laws.)

Based on your previous response, what is your expected year of completion for your final degree/indicated # of CPA credit hours?
